ISAACUS Hughes de Crutched Fryers in Civitate London Cadaver Infantis Balsamo conditum, Variisque, Ægyptiaco More, notis characteribusque insignitum. D.D.

Translation:

Isaac Hughes, from Crutched Friars in the City of London, gave the body of a child preserved in balsam and marked, in the Egyptian manner, with letters and characters.
